When to use this template
Beta feedback is the highest-leverage product feedback you'll ever collect — the testers are using a pre-launch version, you can still change anything before the rest of the world sees it, and the cost of fixing problems is much lower than after launch.
Use this template:
- During private beta when you have 10–500 invited testers
- During public beta or early access when anyone can sign up to try the pre-launch
- For specific feature rollouts behind a feature flag, before turning on for everyone
- For internal dogfooding when employees use a pre-release version
- For research-preview tools (think: Anthropic Claude, OpenAI GPT) where testers know it's experimental
Send the form 5–7 days after the tester first accessed the beta — enough time to form impressions, before novelty wears off.
Why anonymous beats identified for beta feedback
Beta testers have a specific bias when feedback is identified: they want to keep getting beta access. That bias produces nice, encouraging, mostly-positive feedback — even when the tester is actually struggling with the product. The result: you launch with the bugs and confusion the testers were too polite to mention.
Anonymous beta feedback removes that incentive. The tester writes what they actually think, including the things that might make them sound critical or like a "difficult tester." You get the unvarnished list of what's broken — exactly what you need before launch.

The 5 questions, explained
1. "Overall impression so far?" (rating, required) — the headline metric. Track across the beta period — is the score moving up as you ship fixes, or staying flat?
2. "What worked well in your testing?" (text, optional) — captures what to protect. The features testers spontaneously praise are the ones you don't break in the rush to ship.
3. "What was confusing, broken, or missing?" (text, optional) — the unfiltered problems list. This is the question that earns the form its keep. Read every response.
4. "Would you keep using this once it launches?" (choice: Definitely / Probably / Not sure / Probably not, optional) — predicts post-launch retention. If "Probably not" dominates, you have a fundamental issue that no amount of polish will fix.
5. "What's the #1 thing we should fix before launch?" (text, optional) — forward-looking, prioritized, actionable. The most useful single question for your engineering team's next sprint.
5 questions is the right size for beta. Testers expect to give feedback, so completion rates are high (60–80%) — but going past 6 questions hurts.
Best practices
- Send via the beta access channel (email, Discord, Slack, in-product banner) — wherever testers expect communication.
- Send 5–7 days after first access, then again at 30 days for longer betas.
- Aim for 50%+ response rate — much higher than typical product surveys because testers expect to give feedback.
- Don't ask "what's your role?" or "company size?" for small beta cohorts — those questions identify testers and reduce honesty.
- Don't reward beta testers based on positive feedback. That creates a "be nice or lose access" dynamic that kills honest input.
- Acknowledge feedback publicly in beta channels: "We're hearing from testers that X is confusing — we're shipping a fix in Y days." Signal that feedback produces action.
What to do with the responses
A working flow during an active beta:
- Daily: read new responses. Beta feedback is time-sensitive — fast turnaround on bug fixes builds trust with testers.
- Weekly: theme review with the product/eng team. Top 5 issues by volume become next week's sprint priorities.
- Mid-beta and end-of-beta: aggregate report. What's the rating trend? Which issues recurred? What's the launch readiness verdict?
- Pre-launch: review final batch of responses. Anything that 3+ testers flagged in the final week is a launch blocker.
- Post-launch: re-survey the original beta testers. The same form, sent after public launch, tells you if your launch changes addressed the beta concerns.
